As older adults’ health care needs change, they deserve to receive care from experts who understand the importance of maintaining optimal health to so patients can live longer, comfortable and productive lives.

ECHN’s Senior Care program is an interprofessional, collaborative network of healthcare professionals and services, created to help those aged 55 and older receive high quality medical care. From routine doctor’s visits to outpatient treatments, to inpatient hospital stays or home care services, our skilled medical team provides compassionate and expertise medical attention, coordination of care as well as provide loved ones and caregivers access to essential information and resources.

Our Senior Care Program services include:

Routine Primary Care Visits
Primary Care Physicians provide medical evaluation and treatment with a focus on overall wellness, chronic disease management and cognitive and functional assessment. We offer Annual Wellness Visits as well that are a part of the Medicare Part B Plan. Annual Wellness Visits allow you and your doctor to establish a personalized prevention plan to help you stay healthy and get the most out of your relationship with your provider.

Geriatric Care Visits
We offer care specific to older adults through a Geriatrician. Geriatricians are trained to treat patients with multiple medical concerns at the same time, and they can provide counseling and resources for caregiver needs.

Specialty Care Visits
ECHN Medical Group is comprised of specialty care services that provide confidence, safety and relief in Cardiology, Gastroenterology, Endocrinology, Orthopedics, and Osteopathic Manipulative Medicine. Your primary care physician or Geriatrician can work with you to determine the best course of medical attention based on your specific medical concerns, concerns or injury.

Cardiac Rehabilitation
Living with a heart condition can be difficult physically and psychologically. Nationally certified through the American Association of Cardiovascular and Pulmonary Rehabilitation (AACVPR), ECHN’s cardiac rehabilitation program combines education and exercise components to help individuals with a variety of cardiac conditions increase overall quality of life and to better manage and prevent future cardiac episodes.

Outpatient Rehabilitation Services
Our rehabilitation program aims to help develop, maintain and restore maximum movement and functional ability. With physical, occupational and speech therapy and specialty-based programs for older adults, including treatments for neurological disorders such as Dementia, Stroke and Parkinson’s disease, our team of licensed, certified and experienced therapists work with you and your healthcare providers to create a unique treatment plan tailored to your individual needs. We also offer home-based treatment and suppport to those who may need it.

ECHN Diagnostics operates a cutting-edge, fully automated laboratory at Manchester Memorial Hospital so test samples can be received, prepared for testing and analyzed on a single line in order to provide physicians with results in a timely fashion. We also provide easy access to routine blood draw and more in-depth testing by operating many walk-in blood draw locations throughout our community. We also offer home-based lab draw to those who may need it.

Behavioral Health Services
Our adult outpatient mental health services are designed to decrease barriers to treatment by providing a variety of counseling options including individual therapy, specialized group therapies and family counseling. Our team of clinicians will complete a comprehensive assessment and talk to you in more depth about services and available programs.

Those aged 55 and older who are experiencing emotional and behavioral changes that cannot be managed in an outpatient setting, ECHN’s Older Adult Behavioral Health Unit located at Rockville General Hospital is an inpatient unit designed to provide a safe and secure environment to address special needs.
