Diabetes Services
ECHN’s diabetes services are comprised of community resources for the diagnosis, education and treatment of diabetes. ECHN offers three distinct care programs for people living with diabetes, each is designed to provide medical care and support to manage the disease and improve quality of life. The best time for diabetes education are:
- When you’re diagnosed with diabetes
- As part of your annual assessment of education, nutrition or emotional issues
- When new complications arise
- During changes in your healthcare team or treatment
Diabetes Self-Management Program
The Diabetes Self-Management Program has been recognized for excellence by the American Diabetes Association, and ECHN has been honored as one of its “most influential partners” in the fight against Diabetes in Connecticut. This program supports and inspires individuals to live full happy lives and includes educational materials, instructions and group education classes.

Led by the Centers for Disease Control and Prevention, PreventT2 is a proven system to help identify the signs of diabetes early and even prevent or delay the onset of type 2 diabetes.

Gestational Diabetes Program
ECHN’s gestational diabetes program has been recognized for excellence by the American Diabetes Association repeatedly since 1996 and has been honored by the American Diabetes Association as one of its “most influential partners” in the fight against diabetes throughout Connecticut. Certified diabetic educators work with pregnant women to create a personalized self-management plan that meets individualized needs and lifestyles. The focus is on creating a personalized care plan for you that includes ample support and education.

When you or someone you love has been diagnosed with diabetes, the impact it can have on your life and your family can be overwhelming. Many have found it helpful to attend a support group where they can gain support and share their experiences as a community. Sweet Talk is a FREE diabetes support group. Attending a diabetes support group is a proactive way to find tips and new ideas to improve your health.
